Elderly Waiver
Preventing Early Nursing Home Placement
The Elderly Waiver program is designed to keep individuals over the age of 65 in their homes safely and to prevent early nursing home placement.
MCPHD provides case management services for individuals in Marion County.
A case manager from MCPHD will complete a comprehensive assessment and intake to assist in determining eligibility for the waiver. If approved, the individual will receive an individualized service plan that addresses the needs of the individual to remain safely in his/her home.
The case manager will set up, coordinate, and monitor these services and meet with the individual at least quarterly to review their needs.
In addition to making the consumer eligible for Medicaid, the following are services that can be funded by the Elderly Waiver program:
- Transporation, Community Van
- Assistive Devices
- Personal Emergency Response Button System
- Home-delivered Meals
- Liquid supplements (Boost, Ensure...)
- Homemaker Service
- Respite
- Mental Health Outreach
- Chore Service
- Home and Vehicle Modification
- Nurtitional Counseling
- Home Health Nursing
